I, the undersigned, acknowledge and agree that participation in athletic activities involves inherent risks, including the risk of injury. By signing below, I voluntarily assume all risks associated with my participation.

I hereby release and hold harmless Game Time & Associated Programs, its employees, volunteers, and affiliates from any and all liability, claims, or demands arising from injuries, damages, or losses sustained as a result of my participation in Youth Holiday Basketball Camp.

I confirm that I am physically capable of participating in this activity and have no conditions that would limit my involvement.
